  • Complete the School Income Inquiry Form for 2025-2026

    Support School Funding-School Income Inquiry forms.

    School lunches are provided free of charge to all students, and you do not need to apply for your child to receive meals. However, we kindly request that families complete the Family Income Inquiry Form.

    This form can help our school receive funding for various programs. 

    Families can  now visit https://www.myschoolapps.comOpens in a new browser tab to conveniently complete the form online.

    Paper forms have been sent home and are also available in the main office.

  • Is your child being picked up by a neighborhood afterschool provider?

    For your child’s safety, it is important that the school is notified of people and programs that can pick up your children at the end of the day in a timely manner.

    Parents/guardians who are having their children picked up after school by outside programs will need to notify the school by printing and completing the attached release form. Please return the form to your child’s teacher, to the main office or Ms. Deise Kowalski, Parent Coordinator.

    Please note: 

    -If your child is starting a new program semester or switching programs you will need to complete a new form. 

    -Forms should be submitted in advance.  ( No later than one school day prior to child beginning their program.) *** If you are making changes to pick up for the same day, please make sure to call the school main office and to contact your child's teacher.***

    - The Online form will not be available between 12-2:30pm during the school week.

    -Hard copies are also available in the main office.

  • Annual Family Educational Rights and Privacy Act (FERPA) Notification for School Year 2025-2026

    The Family Educational Rights and Privacy Act (FERPA) gives parents and students age 18 and older rights over student education records. The Parents’ Bill of Rights for Data Privacy and Security(Open external link) provides you with additional rights, and Chancellor’s Regulation A-820(Open external link) provides additional information.

    Local Afterschool Programs

    The PTA has compiled a list of local afterschool programs http://www.ps34.org/groups/4090/ps_34_pta/afterschool
    This collection offers a variety of options for families. Check back often for updates.

    School Settlement Association Grades k-5
    School Settlement Association at PS34 is a school-based program that only enrolls PS34 participants in grades k-5. Families can now begin to sign up online for the 2023-2024 school year. The program runs from the end of the school day to 5:45 pm.  
     
    How to Apply:

    Step 2 – Search for the program school (PS34) https://discoverdycd.dycdconnect.nyc/program-public
    Step 3 - Complete the application and submit.
     
    Please note that application submission does NOT mean your child/ren are enrolled in the program. You will receive an acceptance letter via email from the Assistant Director Jose Flores to confirm that you have been accepted into the program.

    Get your NYC Schools Account (NYCSA) 

    With a NYC Schools Account current Department of Education families can see their children’s academic information, such as grades and attendance, and confirm your address and information. We can help you create your account once enrolled.

Picture Days will be on:
Friday, October 10: for individual portraits.
Tuesday, October 14: for class photos & 5th grade senior photos.

There are no forms to fill out prior to Picture Days.  All students present will have their photos taken.

New Cell Phone & Electronic Device Policy

Under the updated policy, Chancellor's Regulation A-413, students are not permitted to use personal internet-enabled electronic devices during the school day. This includes devices such as cell phones, laptops, tablets, and portable music and entertainment systems.

Beginning in the 2025-26 school year, in accordance with New York State law,(Open external link) New York City Public Schools (NYCPS) is updating its policy regarding the use of personal internet-enabled electronic devices—including cell phones—in school. This change supports the State's intent to create distraction-free schools(Open external link) and aims to ensure safe and focused learning environments across all NYCPS schools.
